GUIDELINES FOR NEWLY ADMITTED STUDENTS OF MBBS & BDS

1.      Deposit your leftover dues, if any, in Accounts Department by 2 Dec 2011.

2.      Get your Uniform prepared by 7 Dec 2011.

3.      Submit following documents in the Department of Medical Education by  8 Dec 2011:-

          a.      Photocopy of Fee Deposited Voucher by Accounts Department.

b.       Original Migration Certificate if migrated form Board, other than Punjab and Federal Board.

          c        3x Copies of all Academic Documents (Matric / FSc or Equivalent Certificate

d.     20 x photographs 4 x 4 cm with blue background.

e.     06x photographs 1” x 1” with blue background.

f.       02x Photocopy of result of UHS entry test.

g.      02x Photocopy of CNIC of Student and father.

4.      Fill in the following and submit in the dept of Medical Edn as under:-.

          a.      Personal Bio data form 8 Dec 2011.

          b.      Undertaking 8 Dec 2011.
